Please review the following information regarding the travel clinic services.

• Research, utilizing information from Center for Disease control is done by an Intravene nurse to determine what vaccines are recommended or required for travel to specific areas of the world. TYPICALLY, MOST TRAVELED AREAS REQUIRE HEPATITIS A, HEPATITIS B, TETANUS, AND TYPHIM. HOWEVER, SOME AREAS REQUIRE ADDITIONAL VACCINES SUCH AS POLIO, M/M/R, YELLOW FEVER, RABIES, MENINGOCOCCAL, AND JAPANESE ENCEPHALITIS.
• A brief medical and vaccination history is obtained from the traveler.
• This information is reviewed by an Intravene physician with appropriate approval or changes. Prescriptions will be written and signed by your primary care physician or an Intravene physician for all injections and any oral prescriptions needed.
• The traveler is notified of approval of recommended vaccines and an appointment will be made at appropriate intervals if time allows to safely receive all recommended vaccinations before departure.
• The traveler will receive a completed and stamped (if applicable) International Certification of Vaccination.
• A Travel Medicine Advisor information sheet will be given to the traveler containing information about the area to which they are traveling.
• The patient's primary care physician will be notified by letter of all vaccinations given if applicable.

After reviewing the above information, to utilize Intravene services please fill out the Travel Clinic Registration Form and submit to Intravene.

Upon receipt of your registration form an Intravene nurse will call you to finalize arrangements for your vaccines and schedule an appointment. At this time, a non-refundable research fee of $35.00 will be expected.
